Registry Office Certificates.
Certificate from the General Registry of Last Will and Testaments.
Deregistration of the Insured Person as a Public Health System user and/or as a pensioner of the National Social Security Institute or the relevant Regional Government Organisation.
Deregistration of the Insured Person from the Family Booklet.
Certificate testifying the person is still alive.
Municipal cohabitation certificate.
